We are dedicated to providing you with the highest quality, cost effective medical care. We are contracted with many PPO and HMO insurance plans.  Payment is accepted in the form of CASH or CHECK only.  

 

Insurance co-payments and deductibles are due at time of visit.  It is our goal to work with your needs within your insurance guidelines and be an advocate for your care.  However, it is ultimately the patients responsibility to know his/her insurance coverage.
